Tivity Health Tuition Repayment Program Guide

 

At Tivity Health, we value the continuous learning and growth of our employees. In recognition of this, we offer a tuition reimbursement program to support your higher education pursuits. This article provides comprehensive information on how to utilize this benefit.

 

## 1. Eligibility:

All Tivity Health full-time employees with a minimum of 6 months of continuous service are eligible to apply for the tuition reimbursement program.

 

## 2. Covered Expenses:

The program covers direct education-related expenses including:

- Tuition fees
- Mandatory lab fees
- Books and supplies required for coursework

 

## 3. Application Process

To apply for the tuition reimbursement benefit, please follow the steps below:

- **Step 1:** Consult with your Department Manager or HR Representative about your intent to enroll in an academic course.
- **Step 2:** Complete the Tuition Reimbursement Application Form available on the Tivity Health's HR portal.
- **Step 3:** Your application needs to be approved by both your manager and the HR department before you enroll in the course. This process might take up to two weeks.

**Note:** Applications should be submitted at least one month prior to the start of the semester.

 

## 4. Reimbursement Amount

Reimbursement amounts will vary based on degree level:

- Bachelor's Degree Programs: Coverage up to $5,250 per calendar year
- Master's Degree and Doctoral Programs: Coverage up to $8,000 per calendar year

 

## 5. Reimbursement Approval

Upon completion of your course, submit the appropriate paperwork (including receipts and a copy of your grade report) to the HR department. As long as you meet all the requirements and achieve a grade of "C" or higher (or "Pass" in pass/fail courses), you'll receive your reimbursement.

 

## 6. Conditions

- Courses must be taken at an accredited institution and be directly related to your current job or anticipated future opportunities within Tivity Health.
- The course schedule should not interfere with your regular work schedule unless you have approval from your manager.
